1. Place the pump over the penis.
2. The air is pumped (manually or automatically) out of the cylinder, in order to create vacuum.

On the other hand, if you can achieve erection, but cannot keep it long or hard enough, then you don’t need the pump, and can simply use the rubber ring, when your penis become naturally erect.
